• Application Package Completion- Consultant will complete the licensure application its entirety.  Required documents will be attached and package will be ready for agency representative signature to be submitted.
• PAS Policies and Procedure Manual
  • PAS admission packet and all forms needed to provide PAS services.
  • PAS Employee Handbook –include all necessary forms required by licensure and contract guidelines changes within the first year of consultant agreement, the handbook will be updated at no charge.
  • PAS Employee Application Package-include all necessary forms required by licensure and PAS contract programs.
  • QA forms for QAPI  program

• Training – Consultant will provide two training hours for up to 6 staff members. Additional training will be billed at an additional hourly rate.
